1. Download the Datapull Request Excel form from this page (DataPull Request Form - v1.0 - blank.xlsx)
2. Fill out the form with the fields that need to be pulled
   1. each data element required
   2. the module and page location in the APPROACH application where the data element can be viewed
   3. any conditions or filters required
   4. the required date range for the datapull
3. Log into the APPROACH Service Desk (help.approach.org)
4. Select the Datapull Request option
5. Enter the province
6. Fill out all other fields and upload the previously filled out DataPull Request Form
   1. for the Desired Due Date, please be aware that we cannot guarantee that this date can be met, as we are providing datapull requests on a "best effort" basis!
7. Submit the request
8. Once submitted, you will receive a confirmation e-mail
   1. we will then review your request and contact you for any additional information
